Breast Augmentation Techniques

The women of Seattle are among the most beautiful and stylish women in the world. To achieve this image, many women devote a great deal of time and effort to maintaining a healthy and fit physique. However, diet, exercise, and clothing choices can only do so much to mask small breasts. Dr. Marosan uses a specialized technique to perform breast augmentation, which results in less (if any) bruising and less pain, meaning you can recover more quickly. In addition, this technique helps to reduce incidence of capsular contracture. This is accomplished by:

  • Carefully minimizing the amount of disruption to the vascular system to limit bleeding by using a fiberoptic light source and pinpoint coagulation
  • Using a surgical-grade antibiotic cleanser, special skin preparation before surgery, and irrigation of the implant with a triple antibiotic solution to minimize the risk of infection.

Incision Placement Options

Many factors affect the placement of the incision in breast augmentation surgery, such as breast implant type, degree of augmentation, and your particular anatomy. Regardless of which incision site is used, the goal is always to minimize visible scarring. Dr. Marosan will take into consideration your personal circumstances to help you determine which of the following options is appropriate for you.

  • A periareolar incision is made along the bottom border of the areola - the more darkly pigmented circle of skin surrounding the nipple. Although the incision is directly on the breast, in most cases this method will create very little noticeable scarring while providing direct access to the interior breast anatomy. This is chosen by the majority of Dr. Marosan's patients.
  • An inframammary incision is made just below the breast where it connects with the chest wall. This approach provides the surgeon the most direct route for implant placement and adjustment. It is hidden within or slightly above the fold of the breast.
  • A transaxillary incision is made in the crease of the underarm, so scarring is not very visible. However, this incision is not hidden beneath a bra or bikini top, and the incision generally cannot be used again for secondary surgeries.It is best for saline implant or small silicone gel implant placement.


Implant Size Choices

Choosing the right size implants can seem intimidating, but Dr. Marosan and his nurses help make the process comfortable and reliable. There is no absolute "right size" but instead there is a range of sizes that will look proportional and attractive on a woman's figure. Dr. Marosan takes specific breast measurements in order to precisely identify nipple position, breast asymmetries, and, most importantly, the optimal width for each patient's breast implants. Then the patient puts on a surgical bra and "tries out" her new breast fullness using breast forms or "sizers." These sizers give a great preview of breast fullness for a specific implant size (e.g., 380 cc's). Dr. Marosan provides feedback and advice on breast implants for Seattle and Bellevue patients.

Implant Placement Options

Breast implants may be inserted under the pectoral muscle (submuscular or subpectoral placement), subfascial (under the pectoral muscle's tough cover, but above the muscle) and  over the pectoral muscle (subglandular). Placement of the implant depends on many factors, including an individual's anatomy, the degree of breast enlargement desired, and the type of breast implants our Seattle and Bellevue patients choose. Dr. Marosan will help you weigh these considerations, and will make a recommendation for the placement which is appropriate for you.

  • Submuscular placement positions the implant behind the pectoralis major (chest) muscle.Since most surgeons do not place the implant under total muscle coverage, this is also known as a dual plane augmentation. Both saline and silicone gel implants are routinely inserted using this method, which generally provides more tissue coverage superiorly and a more natural appearance for most body types. Patients may experience a slightly longer, less comfortable recovery compared to the subglandular method. There is less interference with mammograms with submuscular implants. Dr. Marosan recommends this placement for more than 75% of his patients.
  • The subglandular method places the implant directly behind the breast tissue, but above the pectoral muscle. Because there is no cutting of the muscle, recovery is usually more comfortable. Dr. Marosan may recommend for women with some specific breast shapes. It is sometimes an option for women who may otherwise need a breast lift, but who wish to avoid the scars or a lift. However, this placement method may make implants appear more noticeable, particularly for women with small breasts or thin skin.



Dr. Marosan and the staff at his Seattle / Bellevue area breast enlargement practice make every effort to ensure your comfort and health before, during, and after surgery. To that end, intravenous anesthesia (or twilight anesthesia) is always administered by a highly qualified CRNA.  Newer anesthesia techniques have improved the recovery time from anesthesia, with few patients experiencing after effects such as nausea and grogginess. Breast implant surgery usually takes about 1-11/2 hours  for Dr. Marosan to perform.
